Chemistry

Sodium Acetate: C~2~H~3~NaO~2~ 3H~2~O. Mw: 136.08. Acetic acid, sodium salt, trihydrate. CAS-6131-90-4; CAS-127-09-3 (anhydrous).

Pharmacologic Category

Replacement Preparations; Electrolyte Solutions. Parenteral Electrolyte Supplement. (ATC-Code: B05XA08).

Mechanism of action

Sodium is the principal extracellular cation. Functions in fluid and electrolyte balance, osmotic pressure control, and water distribution. Acetate is metabolized to bicarbonate which neutralizes hydrogen ion concentration and raises blood and urinary pH.

Therapeutic use

Sodium source in large volume I.V. fluids to prevent or correct hyponatremia in restricted intake. To counter acidosis through conversion to bicarbonate.

Contraindications

Alkalosis, hypocalcemia, low sodium diets, edema, cirrhosis.

Warnings and precautions

Close monitoring of serum sodium concentrations is needed to avoid hypernatremia. Use with caution in acid/base alterations (contains acetate), in edematous patients, and in severe hepatic/renal impairment. Use extreme caution in heart failure. Solution for injection contains aluminum (caution if impaired renal function or premature infants). Avoid extravasation.
