Pharmacologic Category
Diagnostic Agents; Pancreatic Function. (ATC-Code: V04CK01).
Mechanism of action
Human and porcine secretin are both synthetically-derived products and are equally potent on osmolar basis. A hormone normally secreted by duodenal mucosa and upper jejunal mucosa. Increases volume and bicarbonate content of pancreatic juice. Stimulates flow of hepatic bile with high bicarbonate concentration. Stimulates gastrin release in Zollinger-Ellison syndrome.
Therapeutic use
Secretin-stimulation testing to aid in diagnosis of pancreatic exocrine dysfunction. Diagnosis of gastrinoma (Zollinger-Ellison syndrome). Facilitation of endoscopic retrograde cholangiopancreatography visualization.
Pregnancy and lactiation implications
Reproduction studies not conducted. Excretion in breast milk unknown (use caution).
Unlabeled use
Contraindications
Hypersensitivity to secretin or any component of the formulation. Acute pancreatitis.
Warnings and precautions
Potential for allergic reactions exists. Use with caution in hepatic impairment (including ethanol-induced disease; volume response to secretin may be exaggerated). Response may be blunted in presence of inflammatory bowel disease, anticholinergic agents, or following vagotomy (blunted response not indicative of pancreatic disease).
