Chemistry
Lornoxicam: C~13~H~10~ClN~3~O~4~S~2~. Mw: 371.82. (1) 2H-Thieno [2,3-e]-1,2-thiazine-3-carboxamide, 6-chloro-4-hydroxy-2-methyl-N-2-pyridinyl-, 1,1-dioxide; (2) 6-Chloro-4-hydroxy-2-methyl-N-2-pyridinyl-2H-thieno[2,3-e]-1,2-thiazine-3-carboxamide 1,1 dioxide. CAS-70374-39-9 (1993).

Pharmacologic Category
Analgesics and Antipyretics; Nonsteroidal Anti-inflammatory Agents; Cyclooxygenase-2 (COX-2) Inhibitors. Disease-modifying Antirheumatic Drugs. (ATC-Code: M01AC05).
Mechanism of action
Mode of action of lornoxicam partly based on inhibition of prostaglandin synthesis (inhibition of cyclo-oxygenase enzyme COX/PTGS). In vitro inhibition of cyclo-oxygenase does not result in increase in leukotriene formation. Mechanism of analgesic action of lornoxicam not fully determined.
Therapeutic use
Short term treatment of mild-to-moderate pain associated with extraarticular inflammation. Symptomatic treatment of pain and inflammation in osteoarthritis and rheumatoid arthritis.
Pregnancy and lactiation implications
Contraindicated in pregnancy and lactation.
Unlabeled use
Contraindications
Contraindicated in known allergy to lornoxicam or hypersensitivity to any ingredient of the preparation. Hypersensitivity reactions (bronchospasm, rhinitis, angioedema or urticaria) to other NSAIDs, including acetylicsalicylic acid. GI bleeding, cerebrovascular bleeding. Bleeding and coagulation disorders. Active peptic ulceration or history of recurrent peptic ulceration. Severe liver or renal impairment. Thrombocytopenia. Severe or uncontrolled cardiac failure. The elderly (>65 years). Those weighing <50 kg. Those undergoing acute surgery. Those under 18 years of age.
Warnings and precautions
In patients with the following disorders, lornoxicam should only be administered after careful risk-benefit assessment: GI ulceration and bleeding in medical history, previous cerebrovascular hemorrhage, SLE, ulcerative colitis, Crohn’s disease, liver diseases (e.g. liver cirrhosis), porphyria, hematopoietic disorders, or reduced cardiac function. When treating mild-to-moderate cardiac failure, attention must be paid to risk of fluid retention and decreased renal function.
