Alprostadil: C~20~H~34~O~5~. Mw: 354.48. (1) Prost-13-en-1-oic acid,11,15-dihydroxy-9-oxo-;(11α,13E,15S)-; (2)(1R,2R,3R)-3-Hydroxy-2-[(E)-(3S)-3-hydroxy-1-octenyl]-5-oxocyclopentaneheptanoic acid. CAS-745-65-3 (1977).

Pharmacologic Category

Vasodilating Agents, Miscellaneous; Prostaglandin. (ATC-Code: C01EA01; G04BE01; G04BE01).

Mechanism of action

Vasodilation on vascular and ductus arteriosus smooth muscle. Relaxation of trabecular smooth muscle in penile cavernosal arteries.

Therapeutic use

Congenital heart defects in neonates, including ductus arteriosus, pulmonary atresia, pulmonary stenosis, tricuspid atresia, Fallot’s tetralogy, transposition of the great vessels, interruption of aortic arch, coarctation of aorta, hypoplastic left ventricle. Treatment of erectile dysfunction.

Pregnancy and lactiation implications

Unlabeled use

Investigational: Treatment of pulmonary hypertension in infants and children with congenital heart defects with left-to-right shunts.

Contraindications

Hypersensitivity to alprostadil or any component of the formulation. Hyaline membrane disease or persistent fetal circulation. Respiratory distress syndrome. Conditions predisposing to priapism (sickle cell anemia, multiple myeloma, leukemia). Anatomical deformation of the penis. Penile implants.

Warnings and precautions

Apnea may occur in neonates with congenital heart defects (especially in those weighing <2 kg at birth). Priapism may occur when used in erectile dysfunction. Use with caution in neonates with bleeding tendencies.
