Conestat Alpha: C~2355~H~3745~N~613~O~728~S~17~. Human plasma protease C1 inhibitor (C1 esterase inhibitor)(N,O-glycosylated recombinant protein expressed in the mammary gland of transgenic rabbits), glycoform α.

Pharmacologic Category

Other Miscellaneous Therapeutic Agents. Recombinant Human C1 Esterase Inhibitor; Analog of Human C1INH. (ATC-Code: B06AC04).

Mechanism of action

C1INH exerts an inhibitory effect on several proteases (target proteases) of the contact and complement systems. The effect of conestat alpha on target proteases such as activated C1s, kallikrein, factor XIIa and factor XIa, was assessed in vitro. Inhibition kinetics were found to be comparable with those observed for plasma-derived human C1INH. Patients with hereditary angioedema (HAE) have a heterozygous deficiency of the plasma protein C1INH. As a result they may suffer from uncontrolled activation of contact and complement systems, with formation of inflammatory mediators, which clinically becomes manifest as the occurrence of acute angioedema attacks.

Therapeutic use

Treatment of acute angioedema attacks in adults with HAE due to C1 esterase inhibitor deficiency.

Pregnancy and lactiation implications

Not recommended for use during pregnancy or breast-feeding.

Contraindications

Known or suspected allergy to rabbits. Hypersensitivity to the active substance or to any of the excipients.

Warnings and precautions

Conestat alpha is derived from milk of transgenic rabbits and contains traces of rabbit protein. Patients should be tested for the presence of IgE antibodies against rabbit epithelium (dander) prior to initiation of treatment. The possibility of such a cross-reactivity in a patient who has evidence of clinical allergy to cow’s milk cannot be excluded. As with any intravenously-administered protein product, hypersensitivity reactions cannot be excluded.
