Nimodipine: C~21~H~26~N~2~O~7~. Mw: 418.44. (1) 3,5-Pyridinedicarboxylic acid, 1,4-dihydro-2,6-dimethyl-4-(3-nitrophenyl)-, 2-methoxyethyl 1-methylethyl ester; (2) Isopropyl 2-methoxyethyl 1,4-dihydro-2,6-dimethyl-4-(m-nitrophenyl)-3,5-pyridinedicarboxylate. CAS-66085-59-4 (1979).

Pharmacologic Category
Vasodilating Agents, Miscellaneous. Calcium-Channel Blocking Agents; Dihydropyridines. Anti-Dementia Agents; Vasoactive agents. (ATC-Code: C08CA06).
Mechanism of action
Animal studies indicate that nimodipine has greater effect on cerebral arterials than other arterials; this increased specificity may be due to increased lipophilicity and cerebral distribution of the drug as compared to nifedipine. Inhibits calcium ions from entering «slow channels» or select voltage sensitive areas of vascular smooth muscle and myocardium during depolarization.
Therapeutic use
Spasm following subarachnoid hemorrhage from ruptured intracranial aneurysms regardless of patient’s postictus neurological condition.
Pregnancy and lactiation implications
Use in pregnancy only when clearly needed and when benefits outweigh potential hazard to fetus. Teratogenic and embryotoxic effects demonstrated in small animals. No well-controlled studies conducted in pregnant women. Not recommended in nursing women.

Contraindications
Hypersensitivity to nimodipine or any component of the formulation.
Warnings and precautions
Increased angina and/or myocardial infarction has occurred with initiation or dosage titration of calcium channel blockers. Symptomatic hypotension with or without syncope can rarely occur. Intestinal pseudo-obstruction and ileus reported during therapy (use caution in decreased gastrointestinal motility or history of bowel obstruction). The most common side-effect is peripheral edema. Reflex tachycardia may occur. Use with caution in hepatic impairment, or in idiopathic hypertrophic subaortic stenosis. Severe cardiovascular adverse events, including fatalities, resulted.
