Chemistry

Amoxicillin: C~16~H~19~N~3~O~5~S 3H~2~O. Mw: 419.45. (1) 4-Thia-1-azabicyclo[3.2.0]heptane-2-carboxylic acid, 6-[[amino(4-hydroxyphenyl)acetyl]amino]-3,3-dimethyl-7-oxo-, trihydrate[2S-[2α,5α,6β(S*)]]-; (2)(2S,5R,6R)-6-[(R)-(-)-2-Amino-2-(p-hydroxyphenyl)acetamido]-3,3-dimethyl-7-oxo-4-thia-1-azabicyclo[3.2.0]heptane-2-carboxylic acid trihydrate. Pharmacologic Category

Antibacterials; Aminopenicillins. (ATC-Code: J01CA04).

Mechanism of action

Inhibits bacterial cell wall synthesis.

Therapeutic use

Treatment of otitis media, sinusitis and infections caused by susceptible organisms involving the respiratory tract, skin, and urinary tract. Infective endocarditis in surgical or dental procedures. H. pylori eradication.

Pregnancy and lactiation implications

Drug of choice for treatment of chlamydial infections in pregnancy and for anthrax prophylaxis. Clearance is increased during pregnancy. Oral ampicillin-class antibiotics are poorly-absorbed during labor. Caution during lactation.

Unlabeled use

Postexposure prophylaxis for anthrax exposure.

Contraindications

Hypersensitivity to amoxicillin, penicillin, or any component of the formulation.

Warnings and precautions

Serious hypersensitivity (anaphylactoid) reactions have been reported (especially with history of β-lactam hypersensitivity, history of sensitivity to multiple allergens, or previous IgE-mediated reactions). Use with caution in asthmatic patients. Use with caution in renal impairment. Fungal or bacterial superinfection (with prolonged use). Patients with infectious mononucleosis may develop rash (ampicillin-class antibiotics not recommended in these patients). Chewable tablets contain phenylalanine.
