Pharmacologic Category
Nonsteroidal Anti-Inflammatory Agents; Cyclooxygenase-2 (COX-2) Inhibitors. Disease-modifying Antirheumatic Drugs. (ATC-Code: M01AB16; M02AA25).
Mechanism of action
Inhibits cyclooxygenase enzyme (COX/PTGS), thus inhibiting prostaglandin production.
Therapeutic use
Relief of inflammatory and painful disorders. Osteoarthrosis, rheumatoid arthritis, ankylosing spondylitis. Renal colic. Postsurgical and post-traumatic pain. Acute gout attack. Dysmenorrhea. Non-articular rheumatism.
Pregnancy and lactiation implications
Should be administered during the first and second trimester of pregnancy only if necessary. Avoid use in the third trimester of pregnancy. Use not recommended during lactation.
Unlabeled use
Contraindications
Hypersensitivity to acetyl salicylic acid or other NSAIDs that may cause asthma attacks, acute rhinitis or urticaria. Previous episode of gastrointestinal hemorrhage or perforation in relation with use of NSAIDs. Active or recurrent peptic ulcer or gastrointestinal hemorrhage. Severe renal impairment. Severe heart impairment. Use during the 3^rd^ trimester of pregnancy. Lactation.
Warnings and precautions
May cause CNS depression. Should be used with caution in any of the following cases: gastrointestinal hemorrhage, the elderly, ulcerative colitis, Crohn’s disease, hypertension or severe heart impairment, porphyria, or concomitant use of diuretics. The existence of cardiovascular risk factors should be considered previously to long-term use of aceclofenac. Risk of thrombotic events. Risk of allergic, anaphylactic or anaphylactic-like reactions. Use not recommended in children. May alter fertility in women. Use with caution in hepatic impairment (treatment should be discontinued if hepatic function deteriorates or symptoms suggesting hepatic disorder appear). Use not recommended in severe renal impairment.
