Chemistry

Albuterol: C~13~H~21~NO~3~. Mw: 239.31. (1) 1,3-Benzenedimethanol, α^1^-[[(1,1-dimethylethyl)amino]methyl]-4-hydroxy-; (2) α^1^-[(tert-Butylamino)methyl]-4-hydroxy-m-xylene-α,α’-diol. CAS-18559-94-9 (1971).

Albuterol Sulfate: (C~13~H~21~NO~3~)~2~.H~2~SO~4~. Mw: 576.70. α1-[(tert-Butylamino)methyl]-4-hydroxy-m-xylene – α,α’-diol sulfate (2:1). CAS-51022-70-9; CAS-18559-94-9 (1974).

Pharmacologic Category

Sympathomimetic (Adrenergic) Agents; Selective β~2~-Adrenergic Agonists. (ATC-Code: R03AC02).

Mechanism of action

Relaxes bronchial smooth muscle by action on β~2~-receptors.

Therapeutic use

Bronchodilator in reversible airway obstruction due to asthma or chronic obstructive pulmonary disease. Prevention of exercise-induced bronchospasm.

Pregnancy and lactiation implications

Maternal administration of oral or intravenous routes may produce tocolytic effects, fetal tachycardia and hypoglycemia. Safe use as an inhalation during pregnancy.

Unlabeled use

Tocolytic agent (injectable form).

Contraindications

Hypersensitivity to albuterol, adrenergic amines, or any component of the formulation. Tachyarrhythmias. Risk of abortion.

Warnings and precautions

Bronchospasm may occur (rare) with inhaled bronchodilating agents. Immediate hypersensitivity reactions (urticaria, angioedema, rash) have occurred. Not for use as a component of chronic asthma therapy without an anti-inflammatory agent. Elevation in blood pressure and heart rate. Arrhythmias. Use with caution in the following situations: cardiovascular disease (arrhythmia, hypertension or heart failure), diabetes mellitus (may increase serum glucose), glaucoma (may elevate intraocular pressure), hyperthyroidism (may stimulate thyroid activity), hypokalemia (may decrease serum potassium) and in seizure disorder (CNS stimulation/excitation). The use of oral systemic albuterol as a quick-relief medication and chronic use of inhaled β-agonists for long-term control of asthma is not recommended.
