Chemistry

Amlodipine Besylate: C~20~H~25~ClN~2~O~5~ C~6~H~6~O~3~S. Mw: 567.05. (1) 3,5-Pyridinedicarboxylic acid, 2-[(2-aminoethoxy)methyl]-4-(2-chlorophenyl)-1,4-dihydro-6-methyl-, 3-ethyl 5-methyl ester, (±)-, monobenzenesulfonate; (2) 3-Ethyl 5-methyl (±)-2-[(2-aminoethoxy)methyl]-4-(o-chlorophenyl)-1,4-dihydro-6-methyl-3,5-pyridinedicarboxylate, monobenzenesulfonate. Pharmacologic Category

Calcium-Channel Blocking Agents; Dihydropyridines. (ATC-Code: C08CA01).

Mechanism of action

Inhibits calcium ion from entering the voltage-sensitive channels of vascular smooth muscle and myocardium during depolarization, producing a relaxation of coronary vascular smooth muscle and coronary vasodilation. Increases myocardial oxygen delivery in vasospastic angina.

Therapeutic use

Treatment of hypertension, symptomatic chronic stable angina, vasospastic (Prinzmetal’s) angina. Prevention of hospitalization due to angina with documented coronary artery disease.

Pregnancy and lactiation implications

Use in pregnancy only when clearly needed and when the benefits outweigh the potential hazard to the fetus. Administration is not recommended during lactation.

Contraindications

Hypersensitivity to amlodipine or any component of the formulation.

Warnings and precautions

Increased angina and/or myocardial infarction has occurred with initiation or dosage titration of calcium channel blockers. Symptomatic hypotension with or without syncope (rarely). Peripheral edema is common. Reflex tachycardia may occur. Caution in severe aortic stenosis, hepatic impairment, or in idiopathic hypertrophic subaortic stenosis.
