Pharmacologic Category

Hormones and Synthetic Substitutes; Adrenals; Corticosteroids. Skin and Mucous Membrane Agents; Anti-inflammatory Agents. (ATC-Code: A07EA04; C05AA05; D07AC01; D07XC01; H02AB01; R01AD06; R03BA04; S01BA06; S01CB04; S02BA07; S03BA03).

Mechanism of action

Controls the rate of protein synthesis, depresses the migration of polymorphonuclear leukocytes and fibroblasts, reverses capillary permeability, and causes lysosomal stabilization at the cellular level to prevent or control inflammation.

Therapeutic use

Anti-inflammatory and immunosuppressant agent (due to its glucocorticoid effects). Inflammatory dermatoses such as seborrheic or atopic dermatitis, neurodermatitis, anogenital pruritus, psoriasis, inflammatory phase of xerosis.

Pregnancy and lactiation implications

Due to its positive effect on stimulating fetal lung maturation, the injection is often used in premature labor (24-34 weeks gestation). Topical products are not recommended for extensive use, in large quantities, or for long periods of time in pregnant women. Use with caution while breast-feeding (do not apply to nipples).

Unlabeled use

Accelerating fetal lung maturation in preterm labor.


Hypersensitivity to betamethasone, other corticosteroids, or any component of the formulation. Systemic fungal infections. Intramuscular administration contraindicated in idiopathic thrombocytopenia purpura.

Warnings and precautions

May cause hypercorticism or suppression of hypothalamic-pituitary-adrenal axis, which may lead to adrenal crisis. Use caution upon withdrawal and discontinuation of the drug and transference from systemic corticosteroids to inhaled products. May increase the incidence of secondary infection, mask acute infection (including fungal infections), prolong or exacerbate viral infections, or limit response to vaccines. Exposure to chickenpox should be avoided. Not for treatment of ocular herpes simplex, cerebral malaria or viral hepatitis. Restrict use in active tuberculosis (only in conjunction with antituberculosis treatment). Possible development of Kaposi’s sarcoma (with long treatment). Acute myopathy reported with high doses. May cause psychiatric disturbances (depression, euphoria, insomnia, mood swings, and personality changes). Discontinuation and/or dose reductions should be gradual. Caution in heart failure (risk of fluid retention and hypertension with prolonged use), diabetes mellitus (may produce hyperglycemia), gastrointestinal disease (perforation risk), hepatic or renal impairment (fluid retention), myasthenia gravis (exacerbation of symptoms, especially during initial treatment), following acute myocardial infarction (risk of myocardial rupture), osteoporosis (risk of increased bone loss and osteoporotic fractures), history of seizure disorder (seizures with adrenal crisis reported), cataracts and/or glaucoma. Metabolic clearance of corticosteroids increases in hyperthyroidism and decreases in hypothyroidism. May affect growth velocity in pediatric patients. Should be used cautiously in the elderly. Occlusive dressings should not be used on weeping or exudative lesions. Should not be used for treatment of rosacea, perioral dermatitis, on face, groin, axillae, or in a diapered area (high potency topical product). Avoid concurrent use of other corticosteroids.
