Chemistry
Butabarbital Sodium: C~10~H~15~N~2~NaO~3~. Mw: 234.23. (1) 2,4,6(1H,3H,5H)-Pyrimidinetrione, 5-ethyl-5-(1-methylpropyl)-, monosodium salt; (2) Sodium 5-sec-butyl-5-ethylbarbiturate. CAS-143-81-7; CAS-125-40-6 (butabarbital).

Anxiolytics, Sedatives, and Hypnotics; Barbiturates. (ATC-Code: N05CA03).
Mechanism of action
Interferes with transmission of impulses from the thalamus to the cerebral cortex resulting in an imbalance in central inhibitory and facilitatory mechanisms.
Therapeutic use
Sedative. Hypnotic.
Pregnancy and lactiation implications
Barbiturates cross the placenta and can be found in fetal tissues. Acute withdrawal symptoms may occur in the neonate following in utero exposure near term (they may be delayed for up to 14 days after birth). Use with caution during lactation.

Contraindications
Hypersensitivity to barbiturates or any component of the formulation. Porphyria.
Warnings and precautions
May cause CNS depression. Use of hypnotic/sedative agents for sleep has been associated with hypersensitivity reactions including anaphylaxis as well as angioedema. May cause paradoxical responses, including agitation and hyperactivity, particularly with pain, and in pediatric patients. An increased risk for hazardous sleep-related activities has been noted. Caution in depression or suicidal tendencies. Caution in history of drug abuse or acute alcoholism (potential for drug dependency exists). Caution in hepatic or renal impairment. Caution in respiratory disease (risk of respiratory depression). Effects may be potentiated when used with other sedative drugs or ethanol. Caution in the elderly and in children. Some products may contain tartrazine (risk of allergic reactions; more frequent in patients who are sensitive to aspirin). Abrupt cessation may precipitate withdrawal, including status epilepticus in epileptic patients. Rare reports of agranulocytosis. Caution with clozapine and carbamazepine. Enzyme induction effects of barbiturates may decrease effects of psychotropics. CNS-depressant effects of psychotropics may be enhanced by barbiturates.
