Pharmacologic Category

Replacement Preparations. Phosphate-removing Agents. Antidotes. Calcium Salts. Homeopathic products. (ATC-Code: A02AC01; A12AA04).

Mechanism of action

In osteoporosis, it helps to prevent or decrease the rate of bone loss. The calcium in calcium salts moderates nerve and muscle performance and allows normal cardiac function. Combines with dietary phosphate to form insoluble calcium phosphate, which is excreted in feces, and so helps to reduce hyperphosphatemia. Calcium salts as antacids neutralize gastric acidity resulting in increased gastric and duodenal bulb pH.

Therapeutic use

As an antacid. Calcium deficiency or hyperphosphatemia (e.g. mild/moderate renal insufficiency, hypoparathyroidism, osteomalacia, osteoporosis, postmenopausal osteoporosis, rickets).

Pregnancy and lactiation implications

Safe use during pregnancy and breast-feeding.

Unlabeled use

Contraindications

Hypercalcemia, hypophosphatemia, renal calculi. Suspected digoxin toxicity.

Warnings and precautions

Constipation, bloating, and gas are common with calcium supplements. Calcium absorption is impaired in achlorhydria. Hypercalcemia and hypercalciuria are most likely to occur in hypoparathyroid patients receiving high doses of vitamin D. Use caution when administering calcium supplements to patients with history of kidney stones. Calcium administration interferes with absorption of some minerals and drugs. Taking calcium (≤500 mg) with food improves absorption. May produce increased serum calcium and decreased serum magnesium.
