Pharmacologic Category

Antineoplastic Agents; Antimetabolites; Pyrimidine Analogs. (ATC-Code: L01BC06).

Mechanism of action

Capecitabine is a prodrug that undergoes hydrolysis in the liver and tissues to form fluorouracil which is the active moiety. Fluorouracil is a fluorinated pyrimidine antimetabolite that inhibits thymidylate synthetase, blocking the methylation of deoxyuridylic acid to thymidylic acid, interfering with DNA, and to a lesser degree, RNA synthesis. Fluorouracil appears to be phase-specific for the G~1~ and S phases of the cell cycle.

Therapeutic use

Metastatic colorectal cancer. Dukes’ C colon cancer (adjuvant therapy). Metastatic breast cancer.

Pregnancy and lactiation implications

Animal studies have demonstrated teratogenicity and fetal loss. There are no adequate studies in pregnant women, but risk of fetal harm is known. Women of childbearing potential should avoid pregnancy. Not recommended during lactation.

Unlabeled use

Gastric cancer. Pancreatic cancer.

Contraindications

Hypersensitivity to capecitabine, fluorouracil, or any component of the formulation. Known deficiency of dihydropyrimidine dehydrogenase (DPD). Severe renal impairment (CrCl <30 mL/minute).

Warnings and precautions

There has been cardiotoxicity associated with fluorinated pyrimidine therapy, including myocardial infarction, angina, dysrhythmias, cardiogenic shock, sudden death, ECG changes, and cardiomyopathy (higher risk if coronary artery disease). May cause severe diarrhea. May cause hand-and-foot syndrome (palmar-plantar erythrodysesthesia or chemotherapy-induced acral erythema). May cause necrotizing enterocolitis (typhlitis). Use with caution in bone marrow suppression. Rare and unexpected severe toxicity (diarrhea, neutropenia, neurotoxicity, stomatitis) may be attributed to DPD deficiency. Use with caution in hepatic or renal impairment. Use with caution in patients who have received alkylating therapy. Capecitabine may increase the anticoagulant effects of warfarin. Use with caution in patients ≥80 years of age. Use with caution in patients who have received extensive pelvic radiation. Sedation is common and may also cause dizziness or insomnia. Neutropenia is common (use caution with clozapine and carbamazepine). Risk of reversible stomatitis, abnormal taste, and taste disturbance. Capecitabine is clastogenic to human peripheral blood lymphocytes in vitro but is not clastogenic to mouse bone marrow (micronucleus test) in vivo.
