Chemistry

Clocortolone Pivalate: C~27~H~36~ClFO~5~. Mw: 495.02. (1) Pregna-1,4-diene-3,20-dione, 9-chloro-21-(2,2-dimethyl-1-oxopropoxy)-6-fluoro-11-hydroxy-16-methyl-, (6α,11β,16α)-; (2) 9-Chloro-6α-fluoro-11β,21-dihydroxy-16α-methylpregna-1,4-diene-3,20-dione 21-pivalate. CAS-34097-16-0 (1972).

Pharmacologic Category

Skin and Mucous Membrane Agents; Anti-inflammatory Agents. Topical Corticosteroid. (ATC-Code: D07AB21).

Mechanism of action

Stimulates the synthesis of enzymes needed to decrease inflammation, suppress mitotic activity, and cause vasoconstriction.

Therapeutic use

Inflammation of corticosteroid-responsive dermatoses (intermediate-potency topical corticosteroid).

Pregnancy and lactiation implications

Unlabeled use

Contraindications

Hypersensitivity to clocortolone or any component of the formulation. Viral, fungal, or tubercular skin lesions.

Warnings and precautions

Systemic absorption of topical corticosteroids may cause hypothalamic-pituitary-adrenal axis suppression (reversible), particularly in younger children, which may lead to adrenal crisis. Allergic contact dermatitis can occur. Prolonged treatment with corticosteroids has been associated with the development of Kaposi’s sarcoma. Adverse systemic effects (hyperglycemia, glycosuria, fluid and electrolyte changes, and hypothalamic-pituitary-adrenal axis suppression) may occur when used on large surface areas, for prolonged periods, or with an occlusive dressing. Chronic use of corticosteroids in children may interfere with growth and development.
