Chemistry

Cromolyn Sodium: C~23~H~14~Na~2~O~11~. Mw: 512.33. (1) 4H-1-Benzopyran-2-carboxylic acid, 5,5′-[(2-hydroxy-1,3-propanediyl)bis(oxy)]bis[4-oxo-,disodium salt]; (2) Disodium 5,5′-[(2-hydroxytrimethylene)dioxy]bis[4-oxo-4H-1-benzopyran-2-carboxylate]. CAS-15826-37-6; CAS-16110-51-3 (cromolyn)(1968).

Pharmacologic Category

Respiratory Tract Agents; Anti-inflammatory Agents; Mast-cell Stabilizers. EENT Preparations; Antiallergic Agents. (ATC-Code: A07EB01; D11AH03; R01AC0; R03BC01; S01GX01).

Mechanism of action

Inhibits degranulation of sensitized mast cells which occurs after exposure to specific antigens. Inhibits release of mediators of type I allergic reactions, including histamine and cysteinyl leukotrienes from sensitized mast cells. Inhibits phospholipase A-induced mast cell degranulation and subsequent release of chemical mediators in vitro. May block calcium channels in mast cell membranes. No direct antihistamine, anticholinergic, antiserotonin, anti-inflammatory, vasoconstrictor, or corticosteroid-like properties.

Therapeutic use

Prophylaxis of allergic disorders, including asthma. Prevention of exercise-induced bronchospasm. Prevention and treatment of seasonal and perennial allergic rhinitis. Systemic mastocytosis. Vernal keratoconjunctivitis, vernal conjunctivitis, and vernal keratitis.

Pregnancy and lactiation implications

In animal studies, cromolyn has produced adverse effects on the fetus (e.g. increased resorption, decreased fetal weight) only in very high parenteral doses. Should be used during pregnancy only when clearly needed. It is not known if cromolyn is distributed into milk in humans (use with caution).

Unlabeled use

Food allergy. Inflammatory bowel disease.

Contraindications

Hypersensitivity to cromolyn or any component of the formulation. Acute asthma attacks.

Warnings and precautions

Severe anaphylactic reactions may occur rarely. Use with caution in history of cardiac arrhythmias. Transient burning or stinging may occur with ophthalmic use. Use with caution in hepatic/renal impairment. Has no benefit for acute situations. Caution should be used when withdrawing the drug or tapering the dose as symptoms may reoccur.
