Chemistry

Dichlorphenamide: C~6~H~6~Cl~2~N~2~O~4~S~2~. Mw: 305.16. (1) 1,3-Benzenedisulfonamide, 4,5-dichloro-; (2) 4,5-Dichloro-m-benzenedisulfonamide. CAS-120-97-8.

Pharmacologic Category

Antiglaucoma Agents; Carbonic Anhydrase Inhibitors. (ATC-Code: S01EC02).

Mechanism of action

A nonbacteriostatic sulfonamide which inhibits carbonic anhydrase resulting in renal excretion of sodium, potassium, bicarbonate, and water.

Therapeutic use

Adjunct in treatment of open-angle glaucoma and perioperative treatment for angle-closure glaucoma. Has also been used to treat muscle weakness in hypokalemic periodic paralysis resistant to acetazolamide.

Unlabeled use

Contraindications

Hypersensitivity to dichlorphenamide or any component of the formulation. Severe pulmonary obstruction. Severe renal impairment.

Warnings and precautions

Contraindicated in sulfonylurea allergy. Chemical similarities are present among sulfonamides, sulfonylureas, carbonic anhydrase inhibitors, thiazides, and loop diuretics (except ethacrynic acid) and a risk of cross-reaction exists in allergy to any of these compounds (avoid use when previous reaction has been severe).
