Chemistry

Dicyclomine Hydrochloride: C~19~H~35~NO~2~ HCl. Mw: 345.95. (1) [Bicyclohexyl]-1-carboxylic acid, 2-(diethylamino)ethyl ester, hydrochloride; (2) 2-(Diethylamino)ethyl [bicyclohexyl]-1-carboxylate hydrochloride. CAS-67-92-5; CAS-77-19-0 (dicyclomine).

Pharmacologic Category

Anticholinergic Agents; Antimuscarinics/Antispasmodics. (ATC-Code: A03AA07).

Mechanism of action

Blocks action of acetylcholine at parasympathetic sites in smooth muscle, secretory glands and CNS.

Therapeutic use

Treatment of functional bowel/irritable bowel syndrome.

Pregnancy and lactiation implications

Teratogenic effects not observed in animal studies. Contraindicated during lactation.

Unlabeled use

Urinary incontinence.

Contraindications

Hypersensitivity to dicyclomine or any component of the formulation. Obstructive diseases of the GI tract. Severe ulcerative colitis. Reflux esophagitis. Unstable cardiovascular status in acute hemorrhage. Obstructive uropathy. Narrow-angle glaucoma. Myasthenia gravis. Breast-feeding. Infants <6 months of age.

Warnings and precautions

May cause drowsiness and/or blurred vision. Diarrhea (may be a sign of incomplete intestinal obstruction). Heat prostration (use caution in hot weather and/or exercise). Psychosis (in sensitivity to anticholinergic effects or at excessive dosages). Caution in cardiovascular disease (e.g. coronary artery disease, tachyarrhythmias, heart failure, hypertension), hepatic impairment, hiatal hernia, hyperthyroidism, autonomic neuropathy, prostatic hyperplasia, renal impairment, ulcerative colitis or the elderly. Serious respiratory reactions, central nervous symptoms, and deaths reported in children (contraindicated in infants <6 months of age).
