Pharmacologic Category

Mechanism of action

Diethylpropion is an amphetamine congener which is used as an anorexigenic agent. The mechanism of action of diethylpropion in reducing appetite appears to be secondary to CNS effects, specifically stimulation of the hypothalamus to release catecholamines into the CNS; anorexiant effects are mediated via norepinephrine and dopamine metabolism.

Therapeutic use

Short-term (few weeks) adjunct in the management of exogenous obesity.

Pregnancy and lactiation implications

Teratogenic effects not observed in animal studies. Crosses the human placenta. Safe use of diethylpropion during pregnancy not established. The drug should be used with caution in nursing women.

Unlabeled use

Contraindications

Hypersensitivity or idiosyncrasy to diethylpropion or other sympathomimetic amines. Advanced arteriosclerosis. Severe hypertension, Pulmonary hypertension. Hyperthyroidism. Glaucoma. Agitated states. History of drug abuse. During or within 14 days following MAO inhibitor therapy. Concurrent use with other anorectic agents.

Warnings and precautions

Amphetamines may impair the ability to engage in potentially hazardous activities. A rare, frequently fatal disease of the lungs, (primary pulmonary hypertension) has occurred in patients receiving some anorexigens. Possible risk of developing valvular heart disease. Avoid stimulants in known serious structural cardiac abnormalities, cardiomyopathy, serious heart rhythm abnormalities, or other serious cardiac problems. Use with caution in diabetes mellitus. Use with caution in hypertension or symptomatic cardiovascular disease, including arrhythmias. Seizures reported (use with caution in history of seizure disorders or epilepsy). Not recommended for use in patients who have used other anorectic agents within the past year. Prolonged use may lead to dependency. Diethylpropion is not recommended for use in children younger than 12 years of age.
