Chemistry

Felodipine: C~18~H~19~Cl~2~NO~4~. Mw: 384.25. (1) 3,5-Pyridinedicarboxylic acid 4-(2,3-dichlorophenyl)-1,4-dihydro-2,6-dimethyl-, ethyl methyl ester, (±)-; (2)(±)-Ethyl methyl 4-(2,3-dichlorophenyl)-1,4-dihydro-2,6-dimethyl-3,5-pyridinedicarboxylate. CAS-72509-76-3; CAS-86189-69-7 (1985).

Pharmacologic Category

Calcium-Channel Blocking Agents; Dihydropyridines. (ATC-Code: C08CA02).

Mechanism of action

Inhibits calcium ions from entering the «slow channels» or select voltage-sensitive areas of vascular smooth muscle and myocardium during depolarization, producing a relaxation of coronary vascular smooth muscle and coronary vasodilation. Increases myocardial oxygen delivery in vasospastic angina.

Therapeutic use

Treatment of hypertension.

Pregnancy and lactiation implications

Potentially, calcium channel blockers may prolong labor. There are no adequate, well-controlled studies in pregnant women. Use not recommended during lactation.

Unlabeled use

Pediatric hypertension.

Contraindications

Hypersensitivity to felodipine, any component of the formulation, or other calcium channel blockers.

Warnings and precautions

Possible hypotension and/or syncope resulting in reflex tachycardia and precipitation of angina pectoris in susceptible patients. Peripheral edema is common. May reduce coronary perfusion resulting in ischemia (use with extreme caution in severe aortic stenosis). Use with caution in heart failure, or compromised ventricular function (particularly when used in combination with a β-adrenergic blocker). Use with caution in hepatic impairment (felodipine clearance may be decreased), or in hypertrophic cardiomyopathy and outflow tract obstruction (reduction in afterload may worsen symptoms).
