Pharmacologic Category
EENT Preparations; Anti-inflammatory Agents; Corticosteroids. Ophthalmic Corticosteroid. (ATC-Code: C05AA06; D07AB06; D07XB04; D10AA01; S01BA07; S01CB05).
Mechanism of action
Corticosteroids suppress the inflammatory response to mechanical, chemical, or immunologic agents. They also inhibit edema, fibrin deposition, capillary dilation, leukocyte migration, capillary proliferation, fibroblast proliferation, deposition of collagen, and scar formation associated with inflammation.
Therapeutic use
Symptomatic relief of corticosteroid-responsive inflammatory conditions of palpebral and bulbar conjunctiva, cornea, and anterior segment of the globe. Treatment of chronic anterior uveitis and corneal injury from burns or penetration of foreign bodies. Used for anti-inflammatory properties in conjunction with appropriate anti-infective therapy in some bacterial infections of the eye.
Pregnancy and lactiation implications
Use with caution in pregnant or nursing women.
Unlabeled use
Contraindications
Hypersensitivity to fluorometholone or any component of the formulation. Viral diseases of cornea and conjunctiva (including epithelial herpes simplex keratitis, vaccinia, and varicella). Mycobacterial or fungal infections of the eye. Untreated eye infections which may be masked/enhanced by a steroid.
Warnings and precautions
May exacerbate severity of viral infections (use caution in history of herpes simplex). Prolonged use may result in glaucoma and injury to optic nerve and lead to or mask secondary ocular infections (e.g. bacterial, viral, fungal). Visual defects in acuity and field of vision might occur. Posterior subcapsular cataracts may form after long-term use. Use with caution in presence of glaucoma (steroids increase intraocular pressure). Perforation may occur with topical steroids in diseases which thin cornea or sclera. Steroid use may delay healing after cataract surgery. Some products contain benzalkonium chloride (may be adsorbed by contact lenses).
