Flutamide and Leuprolide

Chemistry

Flutamide: C~11~H~11~F~3~N~2~O~3~. Mw: 276.21. (1) Propanamide, 2-methyl-N-[4-nitro-3-(trifluoromethyl)phenyl]-; (2) α,α,α-Trifluoro-2-methyl-4′-nitro-m-propionotoluidide. CAS-13311-84-7 (1975).

Pharmacologic Category

Antineoplastic Agents; Endocrine Therapy; Antiandrogen. (ATC-Code: L02BB01).

Mechanism of action

Nonsteroidal antiandrogen which inhibits androgen uptake or binding of androgen in target tissues.

Therapeutic use

Used in combination with a gonadotropin-releasing hormone (GnRH) analog (e.g. goserelin, leuprolide acetate) in treatment of prostate cancer.

Pregnancy and lactiation implications

May cause fetal harm if used in pregnant women.

Unlabeled use

Female hirsutism.

Contraindications

Hypersensitivity to flutamide or any component of the formulation. Severe hepatic impairment. Pregnancy.

Warnings and precautions

Hazardous agent. Patients with hemoglobin M disease, G6PD deficiency, or patients who smoke, are at risk of toxicities associated with aniline exposure (i.e. methemoglobinemia, hemolytic anemia, cholestatic jaundice). Photosensitivity reactions (e.g. erythema, ulceration, bullous eruptions, epidermal necrolysis) reported. Contraindicated in severe hepatic impairment. Liver failure reported (avoid use in patients whose ALT values are greater than twice the upper limit of normal).
