Chemistry

Folic Acid: C~19~H~19~N~7~O~6~. Mw: 441.40. (1) L-Glutamic acid, N-[4-[[(2-amino-1,4-dihydro-4-oxo-6-pteridinyl)methyl]amino]benzoyl]-; (2) N-[p-[[(2-Amino-4-hydroxy-6-pteridinyl)methyl]amino]benzoyl]-L-glutamic acid. CAS-59-30-3.

Pharmacologic Category

Vitamins; Vitamin B Complex. (ATC-Code: B03BB01).

Mechanism of action

Folic acid is a water-soluble, B complex vitamin. Required for nucleoprotein synthesis and maintenance in erythropoiesis. Stimulates white blood cell and platelet production in folate deficiency anemia. Enhances elimination of formic acid, the toxic metabolite of methanol (unlabeled use).

Therapeutic use

Treatment of megaloblastic and macrocytic anemias due to folate deficiency. Dietary supplement to prevent neural tube defects.

Pregnancy and lactiation implications

Deficiency of folic acid during pregnancy may result in fetal harm.

Unlabeled use

Adjunctive cofactor therapy in methanol toxicity (alternative to leucovorin).

Contraindications

Hypersensitivity to folic acid or any component of the formulation.

Warnings and precautions

Not appropriate for monotherapy with pernicious, aplastic, or normocytic anemias when anemia is present with vitamin B~12~ deficiency. Injection contains benzyl alcohol as preservative, associated with «gasping syndrome» in neonates. Resistance to treatment may occur with depressed hematopoiesis, alcoholism, and deficiencies of other vitamins.
