Pharmacologic Category

Antidotes. (ATC-Code: V03AB34).

Mechanism of action

Fomepizole competitively inhibits alcohol dehydrogenase, an enzyme which catalyzes metabolism of ethanol, ethylene glycol, and methanol to their toxic metabolites.

Therapeutic use

Treatment of methanol or ethylene glycol poisoning alone or in combination with hemodialysis.

Pregnancy and lactiation implications

Caution if used during pregnancy or lactation.

Unlabeled use

Treatment of propylene glycol toxicity.

Contraindications

Hypersensitivity to fomepizole, other pyrazoles, or any component of the formulation.

Warnings and precautions

Mild rash and eosinophilia reported. Transient increases in serum transaminase concentrations and eosinophilia reported following repeated dosing. Vein irritation and phlebosclerosis reported following rapid I.V. infusion or I.V. injection. Acidosis and electrolyte imbalances can affect cardiovascular system. Consider potential interaction with alcohol. Use with caution in hepatic impairment (drug metabolized in liver), or renal impairment (drug excreted in urine).
