Pharmacologic Category
Loop Diuretics. (ATC-Code: C03CA01).
Mechanism of action
Inhibits reabsorption of sodium and chloride in ascending loop of Henle and distal renal tubule, interfering with chloride-binding cotransport system, thus causing increased excretion of water, sodium, chloride, magnesium, and calcium.
Therapeutic use
Management of edema associated with CHF, nephrotic syndrome, and hepatic cirrhosis. I.V. furosemide may also be used as an adjunct in treatment of acute pulmonary edema.
Pregnancy and lactiation implications
Generally, use of diuretics during pregnancy is to be avoided due to risk of decreased placental perfusion.
Unlabeled use
Contraindications
Hypersensitivity to furosemide, any component, or sulfonylureas. Anuria. Patients with hepatic coma or in states of severe electrolyte depletion until the condition improves or is corrected.
Warnings and precautions
Excess amounts of loop diuretics can lead to profound diuresis with fluid and electrolyte loss. Asymptomatic hyperuricemia reported. Nephrotoxicity or ototoxicity might occur. Caution in sulfonylurea allergy (use is specifically contraindicated in product labeling). In cirrhosis, avoid electrolyte and acid/base imbalances that might lead to hepatic encephalopathy. Co-administration of antihypertensives may increase risk of hypotension.
