Gatifloxacin: C~19~H~22~FN~3~O~4~ 1½H~2~O. Mw: 402.42. (±)-1-Cyclopropyl-6-fluoro-1,4-dihydro-8-methoxy-7-(3-methyl-1-piperazinyl)-4-oxo-3-quinolinecarboxylic acid, sesquihydrate. CAS-180200-66-2 (1997).

Pharmacologic Category

EENT Preparations; Antibacterials; Quinolone. Ophthalmic Antibiotic. (ATC-Code: J01MA16; S01AX21).

Mechanism of action

Gatifloxacin is a DNA gyrase inhibitor. Like other fluoroquinolones, gatifloxacin inhibits bacterial DNA gyrase and topoisomerase IV. Usually bactericidal.

Therapeutic use

Treatment of conjunctivitis caused by susceptible Corynebacterium propinquum, Staphylococcus aureus, S. epidermidis, Streptococcus mitis, S. pneumoniae, or Haemophilus influenzae.

Pregnancy and lactiation implications

Gatifloxacin shown to be fetotoxic in animal studies. There are no adequate, well-controlled studies in pregnant women. Caution if administered while nursing.

Contraindications

Hypersensitivity to gatifloxacin, other quinolone antibiotics, or any component of formulation.

Warnings and precautions

Hypersensitivity reactions (i.e. dermatologic, vascular, pulmonary, renal, hepatic, and/or hematologic events). Fungal or bacterial superinfection may result (with prolonged use). Contact lenses should not be worn during treatment. May have potential to prolong QT interval (should be avoided in uncorrected hypokalemia, or concurrent administration of other medications known to prolong the QT interval (antipsychotics, tricyclic antidepressants, ziprasidone)).
