Chemistry

Homatropine Hydrobromide: C~16~H~21~NO~3~ HBr. Mw: 356.25. (1) Benzeneacetic acid, α-hydroxy-, 8-methyl-8-azabicyclo[3.2.1]oct-3-yl ester, hydrobromide, endo-(±)-; (2) 1αH,5αH-Tropan-3α-ol mandelate (ester) hydrobromide. CAS-51-56-9; CAS-87-00-3 (homatropine).

Pharmacologic Category

EENT Preparations; Mydriatics; Ophthalmic Cholinergic Antagonist. (ATC-Code: S01FA05).

Mechanism of action

Blocks response of iris sphincter muscle and accommodative muscle of ciliary body to cholinergic stimulation resulting in dilation and loss of accommodation.

Therapeutic use

Used to produce cycloplegia and mydriasis for refraction. Treatment of acute inflammatory conditions of uveal tract. Used for pre- and postoperative states when mydriasis is required. Optical aid in axial lens opacities.

Pregnancy and lactiation implications

Caution when used during pregnancy or lactation.

Unlabeled use

Contraindications

Hypersensitivity to homatropine or any component of the formulation. Primary glaucoma or predisposition to glaucoma (e.g. narrow-angle glaucoma).

Warnings and precautions

Caution in keratoconus (homatropine may produce fixed dilated pupil), Down’s syndrome (predisposed to angle-closure glaucoma), and children with brain damage. Excessive use, especially in children, geriatric patients, or other susceptible individuals, may produce adverse systemic antimuscarinic effects, including delirium, agitation, and coma. May cause increased intraocular pressure. May cause sensitivity to light. May result in fixed pupil dilation in keratoconus. Use with caution in pediatric and elderly patients due to susceptibility to systemic effects. May contain benzalkonium chloride which may be adsorbed by contact lenses.
