Pharmacologic Category
Analgesics and Antipyretics; Other Nonsteroidal Anti-inflammatory Agents. (ATC-Code: C01EB16; G02CC01; M01AE01; M02AA13).
Mechanism of action
Inhibits prostaglandin synthesis by decreasing activity of enzyme cyclooxygenase (COX/PTGS), which results in decreased formation of prostaglandin precursors.
Therapeutic use
Inflammatory diseases and rheumatoid disorders including juvenile rheumatoid arthritis, mild-to-moderate pain, fever, dysmenorrhea. Ibuprofen lysine is for use in premature infants weighing between 500-1500 g and who are ≤32 weeks gestational age to induce closure of clinically-significant patent ductus arteriosus (PDA) when usual treatments are ineffective. Management of pain and swelling.
Pregnancy and lactiation implications
Unlabeled use
Acute migraine headache, ankylosing spondylitis, cystic fibrosis, gout.
Contraindications
Hypersensitivity to ibuprofen, aspirin, other NSAIDs, or any component of the formulation. Perioperative pain in setting of CABG surgery. Ibuprofen lysine contraindicated in preterm infants with untreated proven or suspected infection. Congenital heart disease where patency of ductus arteriosus is necessary for pulmonary or systemic blood flow. Bleeding (especially with active intracranial hemorrhage or GI bleed). Thrombocytopenia. Coagulation defects. Proven or suspected necrotizing enterocolitis. Significant renal dysfunction.
Warnings and precautions
Anaphylactoid reactions might occur. Do not use in asthma, bronchospasm, rhinitis, or urticaria with NSAID or aspirin therapy. May increase risk of aseptic meningitis, especially in systemic lupus erythematosus and mixed connective tissue disorders. Platelet adhesion and aggregation may be decreased. May prolong bleeding time. Anemia might occur. Increased risk of adverse cardiovascular thrombotic events, including MI, stroke, and new onset or worsening of pre-existing hypertension. Use caution with fluid retention, heart failure, or hypertension. May increase risk of GI irritation, inflammation, ulceration, bleeding, and perforation. Use caution with history of GI disease (bleeding or ulcers), concurrent therapy with aspirin, anticoagulants and/or corticosteroids, smoking, use of alcohol, elderly or debilitated patients. NSAIDs may cause serious skin adverse events including exfoliative dermatitis, Stevens-Johnson syndrome, and toxic epidermal necrolysis. Do not administer in aspirin-sensitive asthma (severe bronchospasm might occur). Use caution in other forms of asthma. Use contraindicated for treatment of perioperative pain in setting of CABG surgery. Severe hepatic reactions (e.g. fulminant hepatitis, liver failure) occurred with NSAID use (use with caution in decreased hepatic function). Not recommended for use in advanced renal disease. Long-term NSAID use may result in renal papillary necrosis. The elderly are at increased risk for adverse effects (especially peptic ulceration, CNS effects, renal toxicity). Some products might contain phenylalanine. Withhold for at least 4-6 half-lives prior to surgical or dental procedures.
