Isoproterenol and Phenylephrine

Chemistry

Isoproterenol Hydrochloride: C~11~H~17~NO~3~ HCl. Mw: 247.72. (1) 1,2-Benzenediol, 4-[1-hydroxy-2-[(1-methylethyl)amino]ethyl]-, hydrochloride; (2) 3,4-Dihydroxy-α-[(isopropylamino)methyl]benzyl alcohol hydrochloride. CAS-51-30-9; CAS-7683-59-2 (isoproterenol).

Pharmacologic Category

Sympathomimetic (Adrenergic) Agents; Non-selective β-Adrenergic Agonists. β~1~/β~2~-Adrenergic Agonist. (ATC-Code: C01CA02; R03AB02; R03CB01).

Mechanism of action

Stimulates β~1~- and β~2~-receptors resulting in relaxation of bronchial, GI, and uterine smooth muscle, increased heart rate and contractility, vasodilation of peripheral vasculature.

Therapeutic use

Ventricular arrhythmias due to AV nodal block. Bradyarrhythmias hemodynamically compromised or atropine- and dopamine-resistant bradyarrhythmias. Third-degree AV block until pacemaker insertion.

Pregnancy and lactiation implications

Caution if used intravenously during pregnancy or lactation.

Unlabeled use

Pharmacologic overdrive pacing for torsade de pointes. Diagnostic aid (vasovagal syncope).

Contraindications

Hypersensitivity to sulfites or isoproterenol, any component of the formulation, or other sympathomimetic amines. Angina, pre-existing cardiac arrhythmias (ventricular). Tachycardia or AV block caused by cardiac glycoside intoxication.

Warnings and precautions

Use with caution in cardiovascular disease (including CAD, coronary insufficiency, hypertension, or degenerative heart disease), diabetics, hyperthyroidism, renal impairment, history of seizure disorder, the elderly, and/or those with history of sensitivity to sympathomimetic amines. Excessive or prolonged use may result in decreased effectiveness. Use with caution in sensitivity to sympathomimetic amines. Some formulations contain sulfites (may cause allergic-type reactions).
