Isosorbide Mononitrate: C~6~H~9~NO~6~. Mw: 191.14. (1) D-Glucitol, 1,4:3,6-dianhydro-, 5-nitrate; (2) 1,4:3,6-Dianhydro-D-glucitol 5-nitrate. CAS-16051-77-7.

Pharmacologic Category

Vasodilating Agents; Nitrates and Nitrites. Vasodilator. (ATC-Code: C01DA14).

Mechanism of action

Decreases preload as measured by pulmonary capillary wedge pressure and left ventricular end diastolic volume and pressure. This effect improves congestive symptoms in heart failure and improves myocardial perfusion gradient in coronary artery disease.

Therapeutic use

Long-acting metabolite of vasodilator isosorbide dinitrate used for prophylactic treatment of angina pectoris.

Pregnancy and lactiation implications

Use with caution during pregnancy and lactation.

Unlabeled use

Contraindications

Hypersensitivity to isosorbide or any component of the formulation. Hypersensitivity to organic nitrates. Concurrent use with PDE-5 inhibitors (sildenafil, tadalafil, or vardenafil). Angle-closure glaucoma (intraocular pressure may be increased). Head trauma or cerebral hemorrhage (increase intracranial pressure). Severe anemia.

Warnings and precautions

Severe hypotension can occur. Paradoxical bradycardia and increased angina pectoris can accompany hypotension. Orthostatic hypotension can also occur. Use with caution in volume depletion, hypotension, right ventricular infarctions, and in hypertrophic cardiomyopathy. Avoid concurrent use with PDE-5 inhibitors (e.g. sildenafil, tadalafil, vardenafil).
