Pharmacologic Category
Calcium-Channel Blocking Agents; Dihydropyridines. (ATC-Code: C08CA03).
Mechanism of action
Inhibits transmembrane influx of extracellular calcium ions across membranes of myocardial cells and vascular smooth muscle cells, without changing serum calcium concentrations. Increases myocardial oxygen delivery in vasospastic angina.
Therapeutic use
Management of hypertension (alone or in combination with other classes of antihypertensive agents).
Pregnancy and lactiation implications
Teratogenic effects not observed in animal studies. Israpidine crosses human placenta. There are no adequate studies in pregnant women. Use not recommended during lactation.
Unlabeled use
Pediatric hypertension.
Contraindications
Hypersensitivity to isradipine or any component of the formulation. Hypotension (<90 mm Hg systolic).
Warnings and precautions
Increased angina and/or MI have occurred. Might precipitate or worsen heart failure. Possible symptomatic hypotension with or without syncope. Peripheral edema common. Reflex tachycardia might occur. Use with caution in heart failure, hepatic impairment, or in idiopathic hypertrophic subaortic stenosis. Use controlled release tablets with caution in severe GI narrowing. Doses should be adjusted at 2- to 4-week intervals.
