Chemistry

Mepivacaine Hydrochloride: C~15~H~22~N~2~O HCl. Mw: 282.81. (1) 2-Piperidinecarboxamide, N-(2,6-dimethylphenyl)-1-methyl-, monohydrochloride, (±)-; (2)(±)-1-Methyl-2′,6′-pipecoloxylidide monohydrochloride. CAS-1722-62-9; CAS-96-88-8 (mepivacaine).

Pharmacologic Category

Local Anesthetics. (ATC-Code: N01BB03).

Mechanism of action

Acts by preventing generation and conduction of nerve impulses.

Therapeutic use

Local or regional analgesia. Anesthesia by local infiltration, peripheral and central neural techniques including epidural and caudal blocks. Not for use in spinal anesthesia. Local anesthesia by nerve block, infiltration in dental procedures.

Pregnancy and lactiation implications

Animal reproduction studies not conducted. Has been used in obstetrical analgesia. Use caution during lactation (excretion in breast milk unknown).

Contraindications

Hypersensitivity to mepivacaine, other amide-type local anesthetics, or any component of the formulation.

Warnings and precautions

Anxiety, blurred vision, dizziness, restlessness, tremor, tinnitus, depression, or drowsiness might be early warning signs of CNS toxicity with local anesthetic injection. Local anesthetics associated with rare occurrences of sudden respiratory arrest. Convulsions due to systemic toxicity leading to cardiac arrest reported, presumably following unintentional intravascular injection. Use with caution in cardiovascular disease, hyperthyroidism, hepatic or renal impairment, acutely ill patients, debilitated patients, and in the elderly. Intravascular injections should be avoided.
