Chemistry

Methacholine Chloride: C~8~H~18~ClNO~2~. Mw: 195.69. (1) 1-Propanaminium, 2-(acetyloxy)-N,N,N-trimethyl-, chloride, (±)-; (2)(±)-(2-Hydroxypropyl)trimethylammonium chloride acetate. CAS-62-51-1; CAS-55-92-5 (methacholine).

Pharmacologic Category

Diagnostic Agents; Respiratory Function. (ATC-Code: V04CX).

Mechanism of action

Stimulates muscarinic, postganglionic parasympathetic receptors, which results in smooth muscle contraction of airways and increased tracheobronchial secretions.

Therapeutic use

Diagnosis of bronchial airway hyperactivity.

Pregnancy and lactiation implications

Avoid use in pregnant women unless potential benefit justifies potential risk to fetus. Administration should be within 10 days of onset of menses or within 2 weeks of negative pregnancy test. Use not recommended during lactation.

Unlabeled use

Contraindications

Hypersensitivity to methacholine, other parasympathomimetic agents, or any component of the formulation. Concomitant use of β-blockers. Repeat administration (other than day of challenge test with increasing doses).

Warnings and precautions

Severe bronchoconstriction and reduction in respiratory function can result. Use only in cardiovascular disease, peptic ulcer disease, seizures, thyroid disease, urinary tract obstruction, and vagotonia if benefit outweighs risk. Methacholine is bronchoconstrictor for diagnostic purposes only. Product should not be handled by persons with asthma or hay fever.
