Chemistry

Methscopolamine Bromide: C~18~H~24~BrNO~4~. Mw: 398.29. (1) 3-Oxa-9-azoniatricyclo[3.3.1.0^2,4^]nonane, 7-(3-hydroxy-1-oxo-2-phenylpropoxy)-9,9-dimethyl-, bromide, [7(S)-(1α,2β,4β,5α,7β)]-; (2) 6β,7β-Epoxy-3α-hydroxy-8-methyl-1αH,5αH-tropanium bromide (-)-tropate. CAS-155-41-9.

Pharmacologic Category

Anticholinergic Agents; Antimuscarinics/Antispasmodics. (ATC-Code: A03BB03).

Mechanism of action

Crosses blood-brain barrier and provides peripheral blockade of muscarinic receptors. Reduces volume and total acid content of gastric secretions, inhibits salivation, and reduces gastrointestinal motility.

Therapeutic use

Adjunctive therapy in treatment of peptic ulcer.

Pregnancy and lactiation implications

Scopolamine crosses placenta (fetal toxicity noted). Anticholinergics may suppress lactation.

Unlabeled use

Contraindications

Hypersensitivity to methscopolamine, any component of the formulation, or related drugs. Reflux esophagitis. Glaucoma. Obstructed uropathy. Obstructed disease of gastrointestinal tract (pyloroduodenal stenosis). Paralytic ileus. Intestinal atony of elderly or debilitated individuals. Unstable cardiovascular status in acute hemorrhage. Severe ulcerative colitis. Toxic megacolon. Complicated ulcerative colitis. Myasthenia gravis.

Warnings and precautions

Might cause drowsiness and/or blurred vision. Diarrhea might be sign of incomplete intestinal obstruction. Heat prostration might occur in increased environmental temperature. Use with caution in coronary artery disease, tachyarrhythmias, heart failure, or hypertension; also in hepatic impairment, hyperthyroidism, autonomic neuropathy, prostatic hyperplasia, renal impairment, ulcerative colitis (might precipitate/aggravate toxic megacolon), and in the elderly.
