Midodrine Hydrochloride: C~12~H~18~N~2~O~4~ HCl. Mw: 290.74. (1) Acetamide, 2-amino-N-[2-(2,5-dimethoxyphenyl)-2-hydroxyethyl]-, monohydrochloride, (±)-; (2)(±)-2-Amino-N-(β-hydroxy-2,5-dimethoxyphenethyl)acetamide monohydrochloride. CAS-3092-17-9; CAS-42794-76-3 (midodrine)(1983).

Pharmacologic Category

Sympathomimetic (Adrenergic) Agents; α-Adrenergic Agonists. (ATC-Code: C01CA17).

Mechanism of action

Midodrine forms an active metabolite, desglymidodrine, an α~1~-agonist. This agent increases arteriolar and venous tone resulting in a rise in standing, sitting, and supine systolic and diastolic blood pressure in orthostatic hypotension.

Therapeutic use

Orphan drug: Treatment of symptomatic orthostatic hypotension.

Pregnancy and lactiation implications

Increased rate of embryo resorption and decreased fetal weight observed in animal studies. Use during pregnancy should be avoided unless potential benefit outweighs risk to fetus. Excretion in breast milk unknown (use caution).

Unlabeled use

Investigational: Management of urinary incontinence.

Contraindications

Hypersensitivity to midodrine or any component of the formulation. Severe organic heart disease. Urinary retention. Pheochromocytoma. Thyrotoxicosis. Persistent and significant supine hypertension.

Warnings and precautions

Might cause hypertension. Use with caution in diabetes mellitus, hepatic impairment, or urinary retention. Use with caution in visual problems, especially if receiving fludrocortisone. Use not recommended with supine hypertension.
