Pharmacologic Category

Vitamins; Vitamin K Activity. Antidotes. (ATC-Code: B02BA01).

Mechanism of action

Promotes liver synthesis of clotting factors (II, VII, IX, X); however, exact mechanism of this stimulation unknown.

Therapeutic use

Prevention and treatment of hypoprothrombinemia caused by coumarin derivative-induced or other drug-induced vitamin K deficiency, hypoprothrombinemia caused by malabsorption or inability to synthesize vitamin K. Hemorrhagic disease of newborn.

Pregnancy and lactiation implications

Use caution during pregnancy or lactation.

Unlabeled use

Treatment of hypoprothrombinemia caused by anticoagulant rodenticides.

Contraindications

Hypersensitivity to phytonadione or any component of the formulation.

Warnings and precautions

Severe reactions resembling hypersensitivity (e.g. anaphylaxis) reactions occurred rarely during or immediately after I.V. administration (even with proper dilution and rate of administration). Allergic reactions also occurred with I.M. and SubQ injections. Ineffective in hereditary hypoprothrombinemia. Use with caution in renal impairment (including premature infants), and in newborns, especially premature infants (hemolysis, jaundice, and hyperbilirubinemia reported with larger than recommended doses). Injectable products may contain aluminum (may result in toxic levels following prolonged administration). Some dosage forms contain benzyl alcohol («gasping syndrome» in neonates).
