Mechanism of action
Therapeutic use
Prophylaxis (perioperative) and treatment of bleeding due to acquired deficiency (e.g. overdose of vitamin K antagonist) of one or more prothrombin complex coagulation factors II, VII, IX, and X.
Pregnancy and lactiation implications
Moderate-to-significant decreases in fetal body weight observed in some animal studies. Use during pregnancy only if clearly needed. Parvovirus B19 or hepatitis A, (may be present in plasma-derived products) may affect a pregnant woman more seriously than a nonpregnant woman. Excretion in breast milk unknown (use caution).
Unlabeled use
Contraindications
Hypersensitivity to any component of the formulation. Heparin-induced thrombocytopenia type II or known allergy to heparin. Non-life-threatening bleeds in recent myocardial infarction, high risk of thrombosis, or angina pectoris. Untreated disseminated intravascular coagulation. Bleeding associated with hepatic parenchyme disorders, esophageal varices, or major hepatic surgery.
Warnings and precautions
Development of antibodies to one or more human prothrombin factors may occur rarely. Use associated with thrombosis and may be associated with increased risk of disseminated intravascular coagulation and thromboembolic complications including myocardial infarction, in acquired or congenital factor deficiency (use with caution in myocardial infarction, coronary artery disease, hepatic dysfunction, peri/postoperative patients and neonates). Severe hypersensitivity and anaphylactic reactions may rarely occur. Antithrombin levels should be monitored in patients being treated for bleeding as result of chronic liver disease or liver transplantation. Administration of prothrombin complex concentrate may exacerbate underlying hypercoagulable states in recipients of vitamin K antagonists. Formulation contains heparin. Product of human plasma (may potentially contain infectious agents which could transmit disease). Severe hepatic dysfunction, inadequate absorption of vitamin K (e.g. pancreatic disorders, diarrhea) or vitamin K antagonist therapy or overdose may lead to coagulation factor deficiencies.
