Chemistry

Pseudoephedrine Hydrochloride: C~10~H~15~NO HCl. Mw: 201.69. Benzenemethanol, α-[1-(methylamino)ethyl]-, [S-(R*,R*)]-, hydrochloride; (2)(+)-Pseudoephedrine hydrochloride. CAS-345-78-8; CAS-90-82-4 (pseudoephedrine)(1963).

Pseudoephedrine Sulfate: (C~10~H~15~NO)~2~ H~2~SO~4~. Mw: 428.54. (1) Benzenemethanol, α-[1-(methylamino)ethyl]-, [S-(R*,R*)]-, sulfate (2:1). CAS-7460-12-0 (1978).

Pharmacologic Category

Sympathomimetic (Adrenergic) Agents; α- and β-Adrenergic Agonists. (ATC-Code: R01BA02).

Mechanism of action

Directly stimulates α-adrenergic receptors of respiratory mucosa causing vasoconstriction. Directly stimulates β-adrenergic receptors causing bronchial relaxation, increased heart rate and contractility.

Therapeutic use

Temporary symptomatic relief of nasal congestion due to common cold, upper respiratory allergies, sinusitis. Also promotes nasal or sinus drainage.

Pregnancy and lactiation implications

Use during the 1^st^ trimester may be associated with possible risk of gastroschisis, small intestinal atresia, and hemifacial microsomia due to vasoconstrictive effects of pseudoephedrine. Pseudoephedrine excreted in breast milk.

Unlabeled use

Contraindications

Hypersensitivity to pseudoephedrine or any component of the formulation. With or within 14 days of MAOI therapy. Newborns. Breast-feeding.

Warnings and precautions

Use with caution in cardiovascular disease (including hypertension and ischemic heart disease), diabetes mellitus, increased intraocular pressure or angle-closure glaucoma, prostatic hyperplasia and/or genitourinary obstruction, renal impairment, thyroid dysfunction, and in the elderly (may be more sensitive to adverse effects). Some products may contain phenylalanine.
