Chemistry

Reserpine: C~33~H~40~N~2~O~9~. Mw: 608.68. (1) Yohimban-16-carboxylic acid, 11,17-dimethoxy-18-[(3,4,5-trimethoxybenzoyl)oxy]-, methyl ester, (3β,16β,17α,18β,20α)-; (2) Methyl 18β-hydroxy-11,17α-dimethoxy-3β,20α-yohimban-16β-carboxylate 3,4,5-trimethoxybenzoate (ester). CAS-50-55-5.

Pharmacologic Category

Hypotensive Agents, Miscellaneous. Central Monoamine-Depleting Agent, Rauwolfia Alkaloid. (ATC-Code: C02AA02).

Mechanism of action

Reduces blood pressure via depletion of sympathetic biogenic amines (norepinephrine and dopamine). This also commonly results in sedative effects.

Therapeutic use

Management of mild-to-moderate hypertension. Treatment of agitated psychotic states (schizophrenia).

Pregnancy and lactiation implications

Use with caution during pregnancy and in nursing women.

Unlabeled use

Management of tardive dyskinesia.

Contraindications

Hypersensitivity to reserpine or any component of the formulation. Active peptic ulcer disease, ulcerative colitis. History of mental depression (especially with suicidal tendencies). Patients receiving electroconvulsive therapy.

Warnings and precautions

At high doses, significant mental depression, anxiety, or psychosis may occur. Reserpine may cause orthostatic hypotension (use with caution in risk of hypotension or in cardiovascular disease or cerebrovascular disease). Use with caution in asthma, gallstones, inflammatory bowel disease or history of peptic ulcer disease. Use with caution in Parkinson’s disease, and in renal impairment. Avoid concurrent use of MAOIs and/or drugs with MAO-inhibiting properties. Discontinue reserpine 7 days before electroshock therapy.
