Chemistry

Sincalide: C~49~H~62~N~10~O~16~S~3~. Mw: 1143.27. (1) Caerulein, 1-de(5-oxo-L-proline)-2-de-L-glutamine-5-L-methionine-; (2) L-Aspartyl-L-tyrosyl-L-methionylglycyl-L-tryptophyl-L-methionyl-L-aspartylphenyl-L-alaninamide hydrogen sulfate (ester); (3) L-α-Aspartyl-O-sulfo-L-tyrosyl-L-methionylglycyl-L-tryptophyl-L-methionyl-L-α-aspartyl-L-phenylalaninamide. CAS-25126-32-3 (1974).

Pharmacologic Category

Diagnostic Agents; Gallbladder Function. (ATC-Code: V04CC03).

Mechanism of action

Stimulates contraction of gallbladder. Inhibits gastric emptying by causing pyloric contraction, and increases intestinal motility. Stimulates pancreatic secretion. Causes smooth muscle contraction.

Therapeutic use

Postevacuation cholecystography. Gallbladder bile sampling. Stimulation of pancreatic secretion for analysis. Acceleration of transit of barium through small bowel.

Pregnancy and lactiation implications

Fetal harm not observed in animals. There are no adequate studies in pregnant women. Due to its effect on smooth muscle, use during pregnancy should be avoided (may cause spontaneous abortion or premature labor). Use during pregnancy only if clearly needed. Should not be administered near term. Excretion in breast milk unknown (use caution in nursing women).

Contraindications

Hypersensitivity to sincalide or any component of the formulation. Intestinal obstruction.

Warnings and precautions

Some risk of evacuating small gallstones into cystic duct or common bile duct and obstructing flow. Use with caution in pregnancy (may induce premature labor or spontaneous abortion). Solution for injection contains sodium metabisulfite which may cause allergic reactions in some individuals.
