Capreomycin Sulfate: Antibiotic produced by Streptomyces capreolus. CAS-1405-37-4; CAS-11003-38-6 (capreomycin)(1962).

Pharmacologic Category

Antituberculosis Agents. (ATC-Code: J04AB30).

Mechanism of action

Capreomycin is a cyclic polypeptide antimicrobial with bacteriostatic action. The mechanism of action of capreomycin is not well understood. Capreomycin is active in vitro and in vivo against Mycobacterium tuberculosis, M. bovis, M. kansasii, and M. avium. At high concentrations, the drug is also active against some Gram-positive and Gram-negative bacteria.

Therapeutic use

Tuberculosis in conjunction with at least one other antituberculosis agent.

Pregnancy and lactiation implications

Teratogenic in animal studies. Use during pregnancy only if the potential benefit to the mother outweighs the possible risk to the fetus. Use with caution during lactation.

Contraindications

Hypersensitivity to capreomycin or any component of the formulation.

Warnings and precautions

Hypocalcemia, hypokalemia, and hypomagnesemia reported. Prolonged use may result in fungal or bacterial superinfection, including C. difficile-associated diarrhea and pseudomembranous colitis. Nephrotoxicity and ototoxicity are the most serious adverse effects (are most likely to occur in renal impairment, in geriatric patients, and in patients who are receiving other nephrotoxic and/or ototoxic drugs). Extreme caution if used in pre-existing auditory or renal impairment. Use with nonantituberculous drugs (i.e. aminoglycoside antibiotics) having ototoxic or nephrotoxic potential should be undertaken only with great caution. Simultaneous administration of parenteral antituberculous agents (e.g. streptomycin) with capreomycin is not recommended (have similar and sometimes irreversible toxic effects, particularly on eighth cranial nerve and renal function). Use with caution in the elderly.
