Cimetidine Hydrochloride: C~10~H~16~N~6~S HCl. Mw: 288.80. 2-Cyano-1-methyl-3-[2-[[(5-methylimidazol-4-yl)methyl]thio]ethyl]guanidine monohydrochloride. CAS-70059-30-2 (1987).

Pharmacologic Category

Antihistamine Drugs; Other Antihistamines. Antiulcer Agents and Acid Suppressants; Histamine H~2~-Antagonists. (ATC-Code: A02BA01).

Mechanism of action

Competitively inhibits histamine at H~2~-receptors of the gastric parietal cells resulting in reduced gastric acid secretion, gastric volume and hydrogen ion concentration reduced. Has a weak antiandrogenic effect.

Therapeutic use

Short-term treatment of active duodenal ulcers and benign gastric ulcers. Long-term prophylaxis of duodenal ulcer. Gastric hypersecretory states. Gastroesophageal reflux. Prevention of upper gastrointestinal bleeding in critically-ill patients. Prevention or relief of heartburn, acid indigestion, or sour stomach.

Pregnancy and lactiation implications

Teratogenic events not observed in animal studies. Should be used during pregnancy only when clearly needed. Not recommended during lactation.

Unlabeled use

Part of a multidrug regimen for H. pylori eradication to reduce the risk of duodenal ulcer recurrence.

Contraindications

Hypersensitivity to cimetidine, any component of the formulation, or other H~2~ antagonists.

Warnings and precautions

Reversible confusional states linked to use (age >50 years and renal or hepatic impairment are thought to be associated). Relief of symptoms does not preclude presence of a gastric malignancy. Use with caution in hepatic/renal impairment. Rapid I.V. administration associated with rare cases of arrhythmia and/or hypotension. Should not be taken if painful swallowing, vomiting with blood, or bloody or black stools. Should not be used in patients <12 years of age.
