Pharmacologic Category

Antifungals; Pyrimidines. (ATC-Code: D01AE21; J02AX01).

Mechanism of action

Fluorinated pyrimidine analog, synthetic antifungal agent. Penetrates fungal cells and is converted to fluorouracil which competes with uracil interfering with fungal RNA and protein synthesis.

Therapeutic use

Adjunctive treatment of systemic fungal infections (e.g. septicemia, endocarditis, UTI, meningitis, or pulmonary) caused by susceptible strains of Candida or Cryptococcus.

Pregnancy and lactiation implications

Teratogenic in some animal studies; however, there are no adequate, well-controlled studies in pregnant women.

Contraindications

Hypersensitivity to flucytosine or any component of the formulation.

Warnings and precautions

Moderate hypoplasia of bone marrow, resulting in anemia, leukopenia, pancytopenia, thrombocytopenia, or agranulocytosis may occur. Eosinophilia and aplastic anemia also reported. Bone marrow toxicity can be irreversible and may be fatal in immunosuppressed patients (use with extreme caution in bone marrow depression, including patients with hematologic disease and those who currently or previously received treatment with radiation or drugs associated with bone marrow depression). Allergic reactions (including anaphylaxis) reported rarely. Use with extreme caution in impaired renal function (increased risk of adverse effects).
