Chemistry
Nateglinide: C~19~H~27~NO~3~. Mw: 317.42. (1) D-Phenylalanine, N-[[trans-4-(1-methylethyl)cyclohexyl]carbonyl]-; (2)(-)-N-[(trans-4-Isopropylcyclohexyl)carbonyl-D-phenylalanine. CAS-105816-04-4 (2000).

Pharmacologic Category
Antidiabetic Agents; Meglitinides. (ATC-Code: A10BX03).
Mechanism of action
Stimulates insulin release from pancreatic β-cells to reduce postprandial hyperglycemia.
Therapeutic use
Management of type 2 diabetes mellitus (non-insulin-dependent) as monotherapy. In combination with metformin or a thiazolidinedione to lower blood glucose in hyperglycemia.
Pregnancy and lactiation implications
Should not be used during pregnancy. Abnormal blood glucose levels associated with higher incidence of congenital abnormalities. Insulin is drug of choice for control of diabetes mellitus during pregnancy. Not recommended in nursing women.
Unlabeled use
Contraindications
Hypersensitivity to nateglinide or any component of the formulation. Diabetic ketoacidosis, with or without coma. Type 1 diabetes mellitus (insulin-dependent).
Warnings and precautions
May cause hypoglycemia. Use with caution in adrenal and/or pituitary impairment (may be more susceptible to glucose-lowering effects), in moderate-to-severe hepatic impairment, and in severe renal impairment (may be more susceptible to glucose-lowering effects). It may be necessary to discontinue nateglinide and administer insulin if patient exposed to stress (i.e. fever, trauma, infection, surgery). Patients not adequately controlled on oral agents which stimulate insulin release (e.g. glyburide) should not be switched to nateglinide or have nateglinide added to therapy. Indicated for adjunctive therapy with metformin (not to be used as substitute for metformin monotherapy). Combination treatment with sulfonylureas not recommended (no additional benefit). Use with caution in the elderly and in malnourished patients (may be more susceptible to glucose-lowering effects).
