Chemistry

Amantadine Hydrochloride: C~10~H~17~N HCl. Mw: 187.71. (1) Tricyclo[3.3.1.1^3,7^]decan-1-amine, hydrochloride. CAS-665-66-7; CAS-768-94-5 (amantadine)(1964).

Pharmacologic Category

Antiparkinsonian Agents, Adamantanes; Dopamine Agonists. (ATC-Code: N04BB01).

Mechanism of action

Antiparkinsonian activity may be due to inhibition of dopamine reuptake into presynaptic neurons or by increasing dopamine release from presynaptic fibers.

Therapeutic use

Treatment of parkinsonism, or drug-induced extrapyramidal symptoms.

Pregnancy and lactiation implications

Teratogenic effects were observed in animal studies. Impaired fertility has also been reported during animal studies and during human in vitro fertilization. Administration during lactation is not recommended.

Unlabeled use

Contraindications

Hypersensitivity to amantadine, rimantadine, or any component of the formulation.

Warnings and precautions

May cause CNS depression. Neuroleptic malignant syndrome may occur (associated with dose reduction or abrupt discontinuation). Dopamine agonists used for Parkinson’s disease or restless legs syndrome have been associated with compulsive behaviors and/or loss of impulse control (causality not yet established). Suicidal ideation reported (may exacerbate mental problems in patients with history of mental illness). Caution in heart failure, peripheral edema, or orthostatic hypotension, eczema, hepatic impairment, renal impairment, seizure disorder, or in the elderly. Avoid use in untreated glaucoma. Use with caution in uncontrolled psychosis or severe psychoneurosis and in patients receiving CNS stimulant drugs. Do not discontinue abruptly when treating Parkinson’s disease.
