Pharmacologic Category

Antifungals; Polyenes. (ATC-Code: A01AB04; A07AA07; G01AA03; J02AA01).

Mechanism of action

Binds to ergosterol, causing cell death in susceptible fungi. Oxidation-dependent stimulation of macrophages.

Therapeutic use

Treatment of severe systemic and central nervous system infections caused by susceptible fungi such as Candida species, Histoplasma capsulatum, Cryptococcus neoformans, Aspergillus species, Blastomyces dermatitidis, Torulopsis glabrata, and Coccidioides immitis. Fungal peritonitis. Irrigant for bladder fungal infections. Used in fungal infection in bone marrow transplantation, amebic meningoencephalitis, ocular aspergillosis, candidal cystitis, chemoprophylaxis, immunocompromised patients at risk of aspergillosis, refractory meningitis, and coccidioidal arthritis.

Pregnancy and lactiation implications

Contraindicated during lactation (excretion in breast milk unknown).

Contraindications

Hypersensitivity to amphotericin or any component of the formulation.

Warnings and precautions

Possibility of anaphylactic reaction. Acute infusion reactions may occur (more common with the first few doses and generally diminishing with subsequent doses). Use with caution in renal impairment. Avoid use with other nephrotoxic drugs. Pulmonary reactions may occur in neutropenic patients receiving leukocyte transfusions.
