Butenafine Hydrochloride: C~23~H~27~N HCl. Mw: 353.93. (1) 1-Naphthalenemethanamine, N-[[4-(1,1-dimethylethyl)phenyl]methyl]-N-methyl-, hydrochloride; (2) N-(p-tert-Butylbenzyl)-N-methyl-1-naphthalenemethylamine hydrochloride. CAS-101827-46-7; CAS-101828-21-1 (butenafine)(1997).

Pharmacologic Category

Antifungals; Benzylamines, Topical. (ATC-Code: D01AE23).

Mechanism of action

Benzylamine antifungal structurally and pharmacologically related to allylamine antifungals (e.g. naftifine, terbinafine). Butenafine exerts fungicidal activity against dermatophytes (active in vitro against Epidermophyton floccosum, Microsporum canis, Trichophyton mentagrophytes, T. rubrum, and T. tonsurans) and other fungi (active in vitro against Malassezia furfur, Candida albicans and C. parapsilosis) by blocking squalene epoxidation (inhibits the squalene monooxygenase (squalene 2,3-epoxidase)), resulting in inhibition of ergosterol synthesis and subsequent weakening of fungal cell membranes.

Therapeutic use

Treatment of tinea corporis (body ringworm), tinea cruris (jock itch), and tinea pedis (athlete’s foot) caused by Epidermophyton floccosum, Trichophyton mentagrophytes, T. rubrum, or T. tonsurans. Treatment of pityriasis (tinea) versicolor caused by Malassezia furfur (Pityrosporum orbiculare or P. ovale).

Pregnancy and lactiation implications

Teratogenic effects not observed in animal studies. Use with caution during lactation.

Contraindications

Hypersensitivity to butenafine or any component of the formulation.

Warnings and precautions

Caution in patients sensitive to allylamine antifungals (e.g. naftifine, terbinafine)(cross-sensitivity to butenafine may exist). Sensitivity or irritation might occur. Use only for topical application to the skin (avoid contact with eyes, mouth, nose, or other mucous membranes; do not use for scalp or nail infections). Contact dermatitis reported rarely. Burning/stinging, itching, and worsening of condition reported.
