Chemistry

Cetirizine Hydrochloride: C~21~H~25~ClN~2~O~3~ 2HCl. Mw: 461.81. (1) Acetic acid, [2-[4-[(4-chlorophenyl)phenylmethyl]-1-piperazinyl]ethoxy]-, dihydrochloride, (±)-; (2)(±)-[2-[4-(p-Chloro-α-phenylbenzyl)-1-piperazinyl]ethoxy]acetic acid, dihydrochloride. CAS-83881-52-1; CAS-83881-51-0 (cetirizine)(1985).

Pharmacologic Category

Second Generation Antihistamines; Histamine H~1~ Antagonist. (ATC-Code: R06AE07).

Mechanism of action

A metabolite of hydroxyzine, it is a selective antagonist of peripheral histamine H~1~-receptors on effector cells in the gastrointestinal tract, blood vessels, and respiratory tract. Tolerance to antihistaminic effect does not usually occur. Does not show appreciable anticholinergic or antiserotonergic effects in animal models.

Therapeutic use

Perennial and seasonal allergic rhinitis and other allergic symptoms including urticaria. Chronic idiopathic urticaria.

Pregnancy and lactiation implications

Not teratogenic in animal studies. Adequate studies not conducted in pregnant women. Use during pregnancy only if clearly needed. Enters breast milk.

Unlabeled use

Contraindications

Hypersensitivity to cetirizine, hydroxyzine, or any component of the formulation.

Warnings and precautions

May cause CNS depression. Use with caution in hepatic or renal impairment. Use with caution in the elderly (more sensitive to adverse effects).
