Chemistry

Citalopram Hydrobromide: C~20~H~21~FN~2~O HBr. Mw: 405.30. (1) 5-Isobenzofurancarbonitrile, 1-[3-(dimethylamino)propyl]-1-(4-fluorophenyl)-1,3-dihydro-, monohydrobromide; (2) 1-[3-(Dimethylamino)propyl]-1-(p-fluorophenyl)-5-phthalancarbonitrile monohydrobromide. CAS-59729-32-7 (1998).

Pharmacologic Category

Antidepressants; Selective Serotonin-reuptake Inhibitors. (ATC-Code: N06AB04).

Mechanism of action

Selectively inhibits serotonin reuptake in the presynaptic neurons and has minimal effects on norepinephrine or dopamine. Uptake inhibition of serotonin is primarily due to the S-enantiomer of citalopram, and explains the antidepressive effect of citaprolam. Displays little to no affinity for serotonin, dopamine, adrenergic, histamine, GABA, or muscarinic receptor subtypes.

Therapeutic use

Depression.

Pregnancy and lactiation implications

Citalopram and its metabolites cross the human placenta. Nonteratogenic effects in the newborn following exposure late in the third trimester include respiratory distress, cyanosis, apnea, seizures, temperature instability, feeding difficulty, vomiting, hypoglycemia, hypo- or hypertonia, hyper-reflexia, jitteriness, irritability, constant crying, and tremor. Exposure after the twentieth week of gestation has been associated with persistent pulmonary hypertension of the newborn. Enters breast milk.

Unlabeled use

Mild dementia-associated agitation in nonpsychotic patients. Smoking cessation. Ethanol abuse. Obsessive-compulsive disorder in children. Diabetic neuropathy.

Contraindications

Hypersensitivity to citalopram or any component of the formulation. Concomitant use with MAO inhibitors or within 2 weeks of discontinuing MAO inhibitors. Concomitant use with pimozide.

Warnings and precautions

Antidepressants increase the risk of suicidal thinking and behavior in children, adolescents, and young adults (18-24 years of age) with major depressive disorder and other psychiatric disorders. May worsen psychosis in some patients or precipitate a shift to mania or hypomania in bipolar disorder. May impair platelet aggregation (increased risk of bleeding, particularly if used concomitantly with aspirin, NSAIDs, warfarin, or other anticoagulants). Has a low potential to impair cognitive or motor performance. May cause or exacerbate sexual dysfunction. Risk of SIADH and hyponatremia. Use with caution in hepatic impairment (decreased clearance and increased plasma concentrations). Use with caution in severe renal impairment. Serotonin syndrome may occur. Caution in elderly patients (risk of hyponatremia). Caution in pregnant patients. May increase the risks associated with electroconvulsive therapy. May cause withdrawal syndrome.
