Chemistry

Econazole Nitrate: C~18~H~15~Cl~3~N~2~O HNO~3~. Mw: 444.70. (1) 1H-Imidazole, 1-[2-[(4-chlorophenyl)methoxy]-2-(2,4-dichlorophenyl)ethyl]-, mononitrate, (±)-; (2)(±)-1-[2,4-Dichloro-β-[(p-chlorobenzyl)oxy]phenethyl]-imidazole mononitrate. CAS-68797-31-9; CAS-27220-47-9 (econazole)(1979).

Pharmacologic Category

Skin and Mucous Membrane Agents; Antifungals; Azoles. Topical Antifungal Agent. (ATC-Code: D01AC03; G01AF05).

Mechanism of action

An imidazole derivative, econazole is a synthetic azole antifungal agent. Alters fungal cell wall membrane permeability. May interfere with RNA and protein synthesis, and lipid metabolism.

Therapeutic use

Treatment of certain dermatophytoses, including tinea corporis (body ringworm), tinea cruris (jock itch), and tinea pedis (athlete’s foot) caused by Epidermophyton floccosum, Microsporum audouinii, M. canis, M. gypseum, Trichophyton mentagrophytes, T. rubrum, or T. tonsurans. Also used for the treatment of pityriasis (tinea) versicolor, a superficial infection caused by Malassezia furfur (Pityrosporum orbiculare or P. ovale), and for the treatment of cutaneous candidiasis caused by Candida albicans.

Pregnancy and lactiation implications

Econazole cream should be used during the 1^st^ trimester of pregnancy only when considered essential to the welfare of the patient and during the 2^nd^ or 3^rd^ trimesters only when clearly needed. Since it is not known whether econazole is distributed into human milk following topical application, it should be used with caution in nursing women.

Unlabeled use

Econazole nitrate has been used effectively for the treatment of vulvovaginal candidiasis. Has also been used with some success as an otic solution for the treatment of otomycoses.

Contraindications

Hypersensitivity to econazole or any component of the formulation.

Warnings and precautions

Irritation. For topical application to the skin only. Should not be applied to the eye or administered intravaginally.
